We collect personal and medical information directly from you whenever possible—via phone, email, online forms, or during your appointment. This includes:

  • Contact and identity details
  • Medical history and test results
  • Information shared during consultations or treatment

We may also collect information from third parties such as your GP, other healthcare providers, laboratories, or treatment funders—with your consent.

Why do we collect this information?

We collect and use your information to:

  • Confirm your identity
  • Provide clinical treatment and care
  • Refer you to specialists or for tests
  • Communicate with your GP (unless you request otherwise)
  • Manage billing and administration
  • Meet legal and reporting obligations

Your information is used only for your care or where required or permitted by law.

By receiving care from CCRG, you consent to the collection, use, and sharing of your personal information as outlined here. You may choose not to share some information, but this could affect the care we’re able to provide.

Our staff are trained to handle personal information safely. Access is limited and monitored. While we take strong precautions, any information sent over the internet is at your own risk.

Health records are retained for at least 10 years as required by law. All digital data is stored securely in New Zealand.

Accessing or correcting your information

You have the right to request access to, or correction of, the personal information we hold about you.

  • Your full name
  • Date of birth
  • The information you’re requesting

We will respond within 20 working days. If we cannot fulfil your request, we’ll explain why and what you can do next under the Privacy Act 2020.
